The Vital Statistics form is what we use to make Death Certificates.  The sooner we get it and can start processing the Death Certificate the more likely we are to get it to the family in a timely manner. However, due to the fact that many families live out of town and don’t get too meet with us for a couple of days and also families often don’t have a lot of the information on hand, it often takes more time than necessary. So here it is for you to print out or look at. It will help us out a lot if this is completed when you come to make funeral arrangements.
